That is a nasty looking toenail. It almost looks deformed. I am pretty sure that it was not supposed to grow back that way. Symptoms would include discoulouration, thickness, ridges on the nail. It's infectious, so he wouldn't want it speading to other toes. A doctor may prescribe a course of antibiotics if the infection is on more than 3 nails. There are over the counter remedies such as nail paint that helps.
