Other causes are wearing tight footwear, stubbing the toe, altered gait, obesity, and hyperhydrosis. There are three main stages of ingrown toenails. The first stage involves mild pain when edema starts at the nail fold region. During the second stage, a fluid discharge will be noticed as well as increased pain. The third stage is where the granulation tissue becomes apparent. The treatment of ingrown toenails makes use of topical applications, antibiotic therapy, and anti-inflammatory drugs to bring down the swelling.<span id="more-58"></span></p>

<p>Congenital disorders like trisomy 13, unna-thost syndrome, pachyonychia condition can all lead to ingrown toenail. Comorbid conditions like hyperthyroidism, acromegaly, diabetes mellitus and pityriasis can all be considered as causative agents for ingrown toenail.</p>
<p><strong>Different stages of ingrown toenail</strong></p>
<p>There are three different stages that are noticed in the ingrown toenail formation. In stage one, one can notice edema, erythema and mild pain at nail fold region especially lateral nail fold region.</p>
<p>At stage two, there will be increased pain and one can notice discharge of fluid from the edge of the nail.</p>
<p>The third stage is the very important stage as the most significant changes takes place. The hypertrophy and the formation of granulation tissue of the lateral nail fold are evident at this stage.</p>
<p><strong>Treatment</strong></p>
<p>In general soaking the affected toenail in the warm water is the best in case there is no bacterial complication. Inserting the cotton piece at the groove edge of the nail will help. In order to remove the granulation tissue formed, silver nitrate topical application can be tried. In case of presence of bacterial infection, antibiotic therapy is must. Antiseptic lotions and ointments can be applied over the affected area. Anti-inflammatory drugs such as steroids can be given to reduce the inflammation.</p>

<p><strong>Toenail Fungus- Etiology and Epidemiology</strong></p>

Apart from the above factors that contribute to the growth of the nail fungus there are also other factors like chronic diseases and disorder that might be there in a patient. Patients with HIV and diabetes are more likely to get these infections. Men suffering from diabetes are about three times more prone for onychomycosis than diabetic females.</p>
<p>Genetic disorders like the Down syndrome and the blood circulation disorders like the Raynaud syndrome could also contribute to the nail fungus infection.</p>
<p>Cushing’s syndrome that produces excess cortisone could also contribute to the rapid growth and infection of the nail fungus. As the age of the person increase they are more susceptible to nail fungus infections.</p>
<p>People working in the food industry, swimming pools, hospitals, nurses, sportsperson, childcare centers are at risk of getting the nail infections caused by the Dermatophytes.</p>

This drug can be used along with other drugs including antibiotics-erythromycin, or certain allergic medications. This drug is taken weekly per month for 2 or 3 months. This is available as Sporanox.</p>
<p><strong>Terbinafine</strong></p>

This drug is often prescribed daily for 12 weeks for toenail fungus and 8 weeks for fingernail fungus. The safety margin of this drug is high. It is found to be more effective with fewer side effects. This available as LAMISIL in the pharmacy and is available against prescription.</p>
<p>The only effective treatment for chronic nail fungus, which does not respond to oral treatment, is to remove the toenail permanently. This will help in the prevention of pain you might have and recurring bacterial infections (as it is contagious, it may spread to neighbors).</p>
<p>Although this surgical treatment sounds extreme, but provide you the permanent remedy for your chronic problem. Do discuss with this procedure with your podiatrist. This procedure is called matrixectomy.</p>
<p>You should abstain from all alcohol beverages while you’re taking any one of these anti-fungal medicines, because the combination can be hard on your liver. Patients with liver or heart disease generally should not take these medications.</p>

Thick toenails can be restored to their original condition in most cases, except when it is hereditary. To start restoring the thick toenail, you must soak the affected toe in a solution of Betadine and warm water. After a few minutes, trim and clean your nail and cover it with a salicylic acid plaster and do not remove for two days. It may take up to four repetitions of this procedure before the thick toenails are reduced to a much healthier and less repulsive state.</p>
<p>Thick toenails</p>
<p>One can notice that some people are having thick toenails. They are discolored also. The discoloration may yellow or brown. Thick toenails are certain to give unpleasant look to the foot. Besides the person will experience discomfort while wearing the footwear?</p>
<p>One may wonder what causes thick toenails. There plenty of causes for thick toe nail. Poor health and inadequate nutrition can lead to thick toenails. Thick toenails can also be due to genetics and fungal infections.</p>
<p>None prefers thick toenails. Numerous home remedies are available; you can try any one remedy to correct this condition. Here a procedure to get cleared from deformed toenail.</p>
<p>To start with, better wash the instruments like nail clipper with antiseptic solution or alcohol. Take two-tea spoon of mild detergent [house hold] or betadine solution and mix it with half gallons of water, preferably warm water. Place the foot with the thick toenail in the detergent mixed water for about ten minutes. Please ensure that the toenail is completely soaked.</p>
<p>Take the cleaned nail clipper and cut the nail affected straight across. Clean the broken pieces of toenail and the debris that are seen around and the side of the nail using nail file and cotton.</p>
<p>Take a plaster [salicylic acid 40 %] and cut it to the size of the affected nail. Place the plaster over the nail. See that the plaster fits perfectly to the nail and then apply the adhesive bandage to secure the bandage in place.</p>
<p>Let the bandage and the plaster remain as it is for two days. Try to keep the toe dry as much as possible. Take care while wearing the foot wears so that the bandage is not disturbed.</p>
<p>Remove the bandage and the plaster after two days. Clean the debris once more while removing the bandage. Better to use nailbrush for cleaning.  One can use the old used toothbrush also for removing the debris.</p>
<p>Now clean the area with warm water. One can use soapy water also for washing the area. Antiseptic solution needs to be applied to the area immediately after washing.</p>
<p>Correction of thick toenail may not be possible with one time salicylic acid treatment. One has to repeat the same procedure minimum of three to four times.</p>
<p>Incase if the thickening and the discoloration of the toenail was due to fungal infection, then you must have to use oral or topical antifungal medications.</p>

<p>It is not all that easy to diagnose the acral lentiginous melanoma in its early stages as they resemble nail streak or bruise at that stage.  This melanoma will look like a narrow but dark stripe below the toenail and hardly visible to the eye.</p>
<p>Usually this type of melanomas is seen under the toenail or thumb nail. But this doesn&#8217;t mean that they cannot be seen under other nails of the toe or finger. In most cases this melanoma is only benign. In case the person notices any streak in the nail in the absence of trauma or a streak that is increasing in its size or the nail that is getting separated from the nail bed, he/she must waste no time in consulting a dermatologist as these can be the beginning of acral litigious melanoma.</p>
<p>In case of non pigmented acral lentiginous melanoma of the toe, there won’t be any streak formation and the melanoma will look like a chronic infection of the nail bed. As the acral litigious melanoma grows in size, it attains irregular shape but the surface of the melanoma is flat, always. The melanoma may have yellow, brown or black color and at times it can be colorless also.</p>
